What's actually inside your lips

Most people assume the lip is a fairly uniform structure. In fact, research published in 2024 by Professor Sebastian Cotofana and a team of international researchers revealed that the lip's internal anatomy is far more organised and complex than previously understood.

Your lips contain 24 distinct internal compartments. There are 12 compartments in the upper lip and 12 in the lower lip. Each compartment has its own defined position, natural volume, and boundary. Those boundaries are formed by thin walls of connective tissue called vertical septations, which run the full depth of the lip tissue from the outer to the inner surface.

One of the most important findings in their research is that the compartment positions are consistent across all people, regardless of age, sex, or ethnicity. The compartments may differ in size from person to person, but where they sit is predictable. That predictability is what makes anatomy-informed technique possible. A trained practitioner can identify your compartment structure before any treatment begins and plan accordingly.

Why is the compartment size what gives lips their natural shape

The 24 compartments aren't all the same size. They follow a consistent gradient: larger in the centre of the lip and gradually smaller toward the corners. This graduated structure produces the characteristic shape of a naturally beautiful lip: central fullness and a gentle taper that defines curves that narrow as they approach the corners of the mouth.

This gradient is not incidental. It's the underlying architecture of the lip, and it's what a natural result should reflect.

When lip filler is placed in a way that respects this gradient with more product in the larger central compartments and less toward the smaller lateral ones, the result mirrors what the anatomy was already doing. It looks natural because it is natural. It's the same shape your lip was already expressing, just enhanced.

Sometimes, an inexperienced or poorly trained practitioner uses a technique that distributes product evenly across the full width of the lip regardless of compartment size.  The lateral areas, which were never designed to hold much volume, receive as much as the centre. The result is a uniform, undifferentiated fullness that doesn't taper, curve, or reflect the natural architecture beneath. This is the unnatural look.  It has nothing to do with how much filler was used. It's a structural issue, not a volume issue.

Why natural results also stay natural for longer

Those internal compartment walls do more than shape the lip. They also keep filler in place.

When filler is deposited within an intact compartment, the surrounding walls contain it. It has no pathway to travel along. But when a technique crosses those internal walls, even partially, even once, it creates a small channel through the tissue. The lips move constantly while talking, eating, smiling, and engaging in every other expression throughout the day. That movement creates changes in internal pressure within the lip tissue. When that pressure acts on the product sitting next to a disrupted compartment wall, it gradually pushes the filler through the existing channel.

This is lip filler migration, and it's why results that looked great immediately after treatment can gradually lose their definition over the following weeks and months. The border softens. The shape blurs. The result drifts away from what it looked like in the clinic.

A technique that works within the compartment structure avoids creating those pathways in the first place. The result not only looks more natural from the start but also holds its shape longer because the product has nowhere to go.

How a practitioner reads your lip anatomy before treatment begins

The compartment boundaries don't require imaging to identify. They're already visible on the surface of your face, but you need to know what you're looking for.

You may have noticed those fine vertical lines around your lips and even mentioned them in your consultation, wondering whether they can be softened. They are anatomical landmarks for the compartments. Each one marks the position of a vertical septation underneath. The compartment walls pull the skin above them slightly inward when the lip is under tension, making those lines visible on the surface.

Ideally, your practitioner should conduct a brief assessment at the start of your consultation. They should ask you to gently pot. This brings the boundaries into clearer definition.  They should be looking for landmarks such as the position of the philtral columns framing the cupid's bow, the fine vertical lines just to either side of centre, and the point where the lower lip transitions toward the corners.  These all map the compartments specific to your anatomy.

The boundaries that good technique never crosses

The first is the vermillion border. This is the defined edge between the coloured lip tissue and the surrounding skin. Product placed beyond this point sits entirely outside the compartment structure. There are no walls to contain it, no boundaries to keep it in place. Migration from this position is essentially guaranteed over time.

The second is the white roll. This is the pale ridge that runs just above the upper lip, immediately below the vermillion border. This area connects compartments horizontally. Injecting here allows the product to spread across compartment boundaries even before any movement or pressure is applied.

A good practitioner understands these compartment structures and doesn't cross either boundary because it makes the difference between a result that holds and one that drifts.
